Ann Hoang is an experienced Credentialled Pharmacist – Diabetes Educator with over 15 years in practice. She graduated with a Bachelor of Pharmacy from the University of Sydney in 2010, became accredited with the Australian Association of Consultant Pharmacy in 2019, and completed a Postgraduate Certificate in Diabetes Education at James Cook University in 2023.

For the past six years, Ann has been a credentialled pharmacist conducting Home Medicines Reviews, working collaboratively with specialists, GPs, nurses, and allied health professionals to optimise patient care. She is passionate about health promotion and diabetes education, with a strong focus on empowering patients, parents, and carers to prevent complications and achieve better health outcomes. She also regularly delivers professional learning and training to NDIS workers and carers to initiate and support best practice in patient care.

Her services include insulin initiation, continuous glucose monitoring training, blood glucose self-monitoring support, and tailored management for type 1, type 2, and gestational diabetes. Ann provides comprehensive medication reviews and education, ensuring patients feel confident and supported in managing their health.

Ann speaks fluent Vietnamese, allowing her to provide culturally sensitive care and education to patients from diverse backgrounds.
