Patient Forms
If you're coming for the first time, please bring:
✓ A referral letter from your GP
✓ Your Medicare card and any concession cards
✓ Any relevant test results including scans, x-rays and blood tests
✓ Online Patient Registration Form
Important Information About Referrals & Medicare Claims:
At Sydney Endocrinology, we want to make the Medicare claiming process as smooth as possible for you. Please take a moment to read our policy regarding referrals and payments:
Referrals
A valid referral from your GP or specialist is required in order to claim a Medicare rebate.
If you forget to bring your referral on the day, that’s okay — simply send it to us afterwards ensuring it is dated before or on the same day as your appointment.
Once we receive the referral, we will process your Medicare claim and the rebate will be paid directly into your nominated bank account.
GP referrals last for 12 months and specialist referrals last for 3 months
Payments
All consultations must be paid in full on the day of the appointment.
We are unable to bulk bill or delay payment if a referral is missing.
This policy applies to all patients, without exception.
