Managing diabetes DURING THE COVID-19 PANDEMIC

As the COVID-19 crisis evolves, things might seem even more uncertain than ever. Your experience of living with diabetes, and the stresses and uncertainties that it brings to your day-to-day life, might mean that you are feeling overwhelmed and concerned by the implications of pandemic on your health and safety.

Our multidisciplinary diabetes team are here to help, providing you with optional services in diabetics, diabetes education, psychology and group mindfullness programs held at the clinic.

Our team-based approach and 24 hour email contact (via diabetes educators and dietitians) enhances diabetes self-management. Weekend, extended hours and teleconference appointments are also available.

What is a credentialled Diabetes Educator (CDE)?

A CDE is a health professional who possesses comprehensive knowledge of and experience in pre-diabetes, diabetes prevention, and diabetes management.

Our CDE Sr Wendy Bryant provide education and support to people with diabetes to understand and manage diabetes, promoting self-management to achieve individualised behavioural and treatment goals that optimise health outcomes.

Our CDEs can be your first point of contact when you want more information, support and/or motivation in the management of your diabetes, and the link between other health professionals.

Who should see a Diabetes Educator?

Diabetes is unlike other conditions - there are a lot of components to the diabetes disease process. Our CDEs provide diabetes education to assist individuals in the following areas:

  • Understanding of Diabetes Mellitus (Type 1, Type 2, Gestational, Impaired Fasting Glucose)
  • Insulin pump therapy
  • Continuous Glucose meters system
  • Physical activity
  • Medication commencement/changes
  • Insulin initiation education
  • Self-management skills

How can a Diabetes Educator help?

If you are living with pre-diabetes, our CDEs can provide you with initial information, what to look out for if something goes wrong and what can happen in the future. The frequency of your visits to a CDE will depend on your diabetes and your blood glucose levels.

For people with Type 1 Diabetes, our CDEs can help you manage your medication, eye care and looking after your feet.

What does a consultation with our CDE generally include?

  • Review of Self-management skills
  • An in-depth lifestyle assessment
  • Medication review (if required)
  • Counselling and advice on behavioral modification
  • Personalized action (as needed)
  • Monitored progress and outcomes through follow-up consultations, as required.

Carbohydrate Counting Program

A diabetes education program focused on equipping adults with type 1 diabetes with skills in carbohydrate counting, insulin dose adjustment, and other diabetes-related topics. This enables patients to match insulin to diet, rather than the other way around, resulting in more flexibility in an individual’s lifestyle, while improving their diabetes control.

Insulin Pump Therapy Program

Our program, specifically developed for Type 1 Diabetes individuals, focuses on the self-management skills in insulin pump therapy and/or an up-date for those already using insulin pump therapy. We also introduce you to Continuous Glucose monitoring, with insulin pump therapy.

Continuous Glucose Monitoring Systems (CGMS) Program

This program provides an explanation of the different CGM systems. Once utilizing the CGMS the program takes the individual through how to obtain the most out of the systems by:

  • Ensuring accuracy

  • Interpreting trend arrows

  • Information required to make self-management changes

RECLAIM Program

Our RECLAIM Program is an investment in your health. Our bespoke, evidence-based multidisciplinary program aims to reverse insulin resistance and prevent diabetes.

Our specialised team individually tailors your program to work with your current lifestyle and preferences. We work with you over 90 days to build your skills, knowledge, and motivation to make small but effective changes to your day-to-day. This allows you to Reclaim your Energy, Confidence, Lifestyle, Activity levels and most importantly, reverse insulin resistance - regaining Insulin sensitivity and Metabolic fitness.
